Release of Liability, Assumption of Risk Agreement & Media Waiver

TERMS:

The risk of injury for the activities involved in the tournament is significant including the potential for permanent paralysis and death, while particular rules, equipment and personal disciplines may reduce the risk, the risk of serious injury does exist, and,
I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the organizers and any staff member or official, their directors, officials, agents, and employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the tournament (“Releasees”) and assume full responsibility for my participation, and,
I agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation in the tournament, I will remove myself from participation and bring such to the attention of the nearest official immediately.
I authorize girls addicted to basketball (g.a.b), its officers, directors, members, agents and media persons that g.a.b elicits to take and use my picture and or video footage for media and marketing purposes.

Each player listed above, including the captain have read the release of liability, assumption of risk agreement, the media waiver on this registration form and the rules and regulations. Each player fully understand its terms. Each player understands that on the first day of the league, each member listed on this registration form will be signing off on the release of liability, assumption of risk agreement and media consent in hard-copy form.
